I have been working as an English teacher for 17 years with experience in all levels and ages from 5 upwards. I believe that audio comprehension and speaking should be taught as early as possible because English is not a phonetic based language. Grammar and Vocabulary can be taught while promoting comprehension and pronunciation from as early as possible. I also consider the needs of the client when assessing what methodology to use (how structured the class is, relevant content, grammatical and vocabulary focus, etc). The goals and motivations of the student are paramount to achieving results and building the students confidence. In one to one classes this is obviously much easier but it is also possible with larger groups.
